A closer look at choline, brain cell membranes, memory and mental performance

Citicoline is one of the more interesting ingredients in the modern nootropics world.

You may also see it called:

CDP-choline

or

Cytidine diphosphate-choline.

Unlike some herbal nootropics, citicoline is a naturally occurring compound involved in important processes within our cells.

It’s involved in the production of phospholipids, which are important components of cell membranes.

Citicoline has therefore attracted research into:

  • Memory
  • Attention
  • Cognitive performance
  • Brain metabolism
  • Neurological recovery

But does taking citicoline actually make healthy people think better?

That’s where the evidence gets interesting.


What Is Citicoline?

Citicoline is a naturally occurring compound involved in the synthesis of phosphatidylcholine, an important phospholipid found in cell membranes.

It can also be described as a source of choline and cytidine-related compounds.

Choline itself is an essential nutrient involved in several biological processes, including neurotransmitter production and cell-membrane structure.

This gives citicoline a biologically plausible role in brain function.

But once again:

Biological plausibility doesn’t automatically equal a cognitive benefit from supplementation.

That’s what human research has to determine.


Why Is Citicoline Interesting as a Nootropic?

There are several reasons.

First, it’s involved in processes important to neuronal cell membranes.

Second, researchers have investigated it in both healthy adults and people experiencing cognitive impairment.

Third, there is some evidence suggesting possible benefits for memory and attention.

But the results aren’t consistent enough to call citicoline a proven cognitive enhancer for everyone.

In fact, a 2024 assessment by the European Food Safety Authority concluded that a cause-and-effect relationship between citicoline consumption and improvement, maintenance or reduced loss of memory had not been established for middle-aged or older adults with age-associated subjective memory impairment.

That’s an important finding for anyone reading supplement marketing.


Citicoline and Brain Cell Membranes

One of the main reasons citicoline attracts scientific interest is its relationship with phospholipid metabolism.

Phospholipids are major components of cell membranes.

Neurons depend on healthy cell membranes for:

  • Cell signalling
  • Communication
  • Transport
  • Maintaining cellular structure

Citicoline participates in pathways involved in producing phosphatidylcholine.

Researchers have therefore investigated whether supplementation could influence brain function.

But again, we need to distinguish the mechanism from the clinical outcome.

Supporting a biological pathway doesn’t automatically mean you’ll experience better memory.


What Does the Memory Research Show?

This is probably the most interesting area.

A randomized, double-blind, placebo-controlled trial investigated citicoline in 100 healthy adults aged 50โ€“85 with age-associated memory impairment.

Participants received:

500 mg citicoline daily

or placebo for:

12 weeks.

The researchers found significantly greater improvements in some secondary memory outcomes in the citicoline group, including episodic memory and composite memory.

That’s encouraging.

But there’s an important catch.

These were secondary outcomes, rather than the study’s primary outcome.

And another study using a higher dose did not reproduce the same memory benefit.

This inconsistency is one reason the evidence remains uncertain.


What Does EFSA Say?

This is particularly relevant for Suppliwell because we want our content to reflect not only positive studies but also regulatory assessments.

In 2024, EFSA reviewed the evidence surrounding citicoline and memory.

They identified three relevant human intervention studies in healthy people.

Only one randomized controlled trial showed a beneficial effect on episodic memory.

That benefit was seen with 500 mg/day for 12 weeks.

Another study using 1,000 mg/day for three months did not show the same effect.

EFSA therefore concluded that a cause-and-effect relationship had not been established.

That’s a very useful reality check.


So Does Citicoline Improve Memory?

The most honest answer is:

Possibly โ€” but the evidence isn’t strong enough to say it reliably does.

There are positive clinical findings.

There are plausible biological mechanisms.

But the results haven’t been consistent enough to establish a general memory-enhancing effect.

So at Suppliwell, we’d classify memory as:

Promising but inconclusive

rather than:

Proven.


What About Attention?

Attention is another area where citicoline has been investigated.

One randomized controlled study examined a beverage containing citicoline and caffeine in 60 healthy adults.

Compared with placebo, the citicoline-caffeine group demonstrated improvements in several measures involving:

  • Concentration
  • Sustained attention
  • Reaction time
  • Information processing

The researchers also observed EEG changes consistent with increased sustained attention.

Interesting?

Absolutely.

But there’s an important limitation:

It was a combination product.

Therefore, we can’t confidently say that citicoline alone caused all of the observed effects.

Caffeine is itself a powerful cognitive stimulant.

This is exactly why ingredient-specific research matters.


Citicoline + Caffeine

This combination deserves a separate section.

We’ve already discussed caffeine and L-theanine in the Suppliwell library.

Citicoline has also been studied in combination with caffeine.

The idea is attractive:

Caffeine โ†’ alertness and stimulation

Citicoline โ†’ potentially supports cognitive processing and neuronal function

But we should be careful not to turn one combination study into a universal claim.

The evidence suggests that the combination may influence attention and cognitive performance.

It does not establish that adding citicoline to every caffeinated drink will improve productivity.


What About Mental Energy?

Citicoline is sometimes marketed as a way of increasing:

mental energy

or

brain energy.

There are biological reasons why researchers have investigated this idea.

Citicoline may influence phospholipid metabolism and other neuronal processes.

But subjective feelings of energy are not the same thing as demonstrated improvements in cognition.

So I wouldn’t describe citicoline as a stimulant.

It isn’t caffeine.

It isn’t designed to produce a conventional “buzz.”

If someone feels more mentally energetic after taking it, that could reflect several factors, including expectancy and individual response.


What About Dopamine?

Citicoline is sometimes discussed in relation to neurotransmitters including dopamine.

Some experimental research suggests citicoline can influence dopamine-related pathways.

However, this doesn’t mean that taking citicoline simply:

“boosts dopamine.”

Neurotransmitter biology is considerably more complicated than that.

We shouldn’t convert an observed biological effect into a simplistic consumer claim.


What About Stroke and Brain Injury?

This is an entirely different area of research.

Citicoline has been investigated extensively in neurological conditions, particularly stroke.

Some studies have suggested potential benefits for cognitive recovery after stroke.

However, clinical trials have not consistently demonstrated major improvements in important outcomes.

For example, a meta-analysis of randomized trials in acute stroke found no significant differences in long-term mortality, dependency or overall effectiveness, although citicoline appeared to have a favourable safety profile.

This is important.

Citicoline should not be marketed as a treatment for stroke.

Anyone experiencing stroke symptoms needs emergency medical care.


What About Vascular Cognitive Impairment?

There is more encouraging evidence in this area.

The IDEALE study investigated oral citicoline in older people with mild vascular cognitive impairment.

Participants receiving citicoline showed improvements in some cognitive measures compared with controls, and the treatment was well tolerated.

Another long-term study following people after ischemic stroke reported better outcomes in attention-executive functions and temporal orientation among citicoline-treated participants.

However, the researchers themselves concluded that larger clinical trials were needed.

These findings are interesting.

But they’re primarily relevant to people with specific neurological or vascular conditions, not necessarily healthy adults.


What About Dementia?

This needs particularly careful treatment.

A systematic review published in 2023 examined whether citicoline could help prevent or slow dementia.

Seven studies involving people with mild cognitive impairment, Alzheimer’s disease or post-stroke dementia were included.

The studies generally reported positive cognitive findings, and the pooled analysis suggested improvements in cognitive status.

However, the authors noted the limitations of the available evidence and that citicoline’s role in clinical practice has not been established.

Another systematic review looking specifically at citicoline as an adjunct to Alzheimer’s treatment found only two retrospective cohort studies.

Although those studies suggested possible cognitive benefits when citicoline was added to cholinesterase inhibitors, the evidence was limited and observational.

Therefore:

Citicoline is not an established treatment for Alzheimer’s disease or dementia.


Is Citicoline a Choline Supplement?

Yes, but there’s an important distinction.

Citicoline contains choline-related material and can contribute to the body’s choline pool.

But citicoline isn’t simply equivalent to taking a standard choline supplement.

Different forms of choline have different chemical properties and metabolic pathways.

That’s why we shouldn’t assume that:

citicoline = all choline supplements.

They’re related, but not identical.


What Dose Has Been Studied?

Human studies have used different doses.

Some of the cognitive research has examined:

500 mg/day

while other research has investigated:

1,000 mg/day or more.

The 100-person healthy older-adult trial that found improvements in some memory outcomes used 500 mg/day for 12 weeks.

However, EFSA’s review highlighted the inconsistency between studies using different doses.

So we shouldn’t conclude:

“More citicoline equals more cognitive benefit.”

There isn’t good evidence for that.


How Quickly Does Citicoline Work?

This is another area where caution is needed.

Some studies investigate relatively short-term cognitive effects.

Others use supplementation over several weeks or months.

There isn’t enough evidence to confidently say:

“Citicoline works after exactly X minutes.”

For a Suppliwell consumer guide, I’d rather avoid giving a precise timeframe that the research doesn’t justify.


Is Citicoline Safe?

Overall, citicoline has generally been well tolerated in clinical studies.

Reported adverse effects have included:

  • Headache
  • Gastrointestinal discomfort
  • Excitability
  • Restlessness

In the systematic review of citicoline as an adjunct treatment for Alzheimer’s disease, reported adverse events included occasional excitability, gastric intolerance and headache, and were described as self-limiting.

The healthy older-adult trial also monitored adverse events, blood pressure and laboratory measures during the 12-week intervention.

As with any supplement, individual responses can vary.


What About Long-Term Use?

This is where we need to distinguish:

“It appears well tolerated in clinical trials.”

from:

“We know everything about lifelong high-dose supplementation.”

We don’t.

Long-term evidence is more limited than short-term evidence.

So sensible supplementation means avoiding unnecessarily high doses and considering the totality of your diet and other supplements.


Who Might Be Most Interested in Citicoline?

Based on the current research, citicoline may be particularly interesting to people researching:

  • Age-associated memory changes
  • Cognitive performance
  • Attention
  • Mental processing
  • Neurological recovery

But the evidence varies considerably between these areas.

For healthy adults, the evidence is considerably less definitive than supplement advertising sometimes suggests.


What About Healthy Young Adults?

This is an important gap.

Much of the more interesting clinical research has involved:

  • Older adults
  • People with memory complaints
  • People with vascular cognitive impairment
  • People recovering from neurological events

That doesn’t necessarily tell us what citicoline will do in a healthy 25-year-old with no cognitive impairment.

We shouldn’t automatically extrapolate.

My Suppliwell Takeaway

Citicoline is a good example of why we need to look beyond supplement marketing.

There is a real biological rationale for studying it.

There are positive human trials.

There are also negative or inconclusive findings.

And regulatory review hasn’t accepted the evidence as sufficient to establish a memory claim.

So my conclusion is:

Citicoline is a biologically interesting compound with promising but inconsistent evidence for memory and certain aspects of cognitive performance. Its benefits appear more convincing in some older or cognitively impaired populations than they do in healthy adults.

That’s a much more useful conclusion than simply calling it a “brain booster.”


Before You Try Citicoline

I’d ask:

  • What am I hoping to improve?
  • Am I already consuming sufficient dietary choline?
  • Am I taking other supplements containing choline?
  • Am I combining it with caffeine?
  • What dose does the product provide?
  • Am I taking medication for a neurological or cardiovascular condition?
  • Am I expecting an immediate effect?

And remember:

A supplement with a sophisticated biochemical mechanism isn’t automatically a sophisticated cognitive enhancer.

The human outcome is what matters.

Citicoline is one of those ingredients where the science is more interesting than the marketing.

It has a legitimate biological role in cellular metabolism and has been investigated extensively in neurological research.

There are encouraging findings around memory and cognitive performance.

But the evidence in healthy adults isn’t strong or consistent enough to describe citicoline as a guaranteed cognitive enhancer.

The most responsible conclusion is:

Citicoline may support certain aspects of cognitive performance, particularly memory in some populations, but the evidence remains mixed and a general cognitive-enhancement effect has not been firmly established.

That’s exactly the kind of distinction we want throughout the Suppliwell library.
